Test kits are available upon request to providers with a US BioTek Account.

If you do not have a US BioTek Account, you can create one here.

Provider located in the state of New York can order US BioTek testing but need to fill out a New York State Non-Permitted Laboratory Test Request Approval Form.

 

Shipping Information

Our test kits are shipped free of charge via USPS or FedEx ground to clients within the US and Canada. Orders shipped outside of the US and Canada are shipped via FedEx Express with costs billed back to the client. These fees vary based on the weight of the shipment. Please allow up to two weeks for fulfillment.
