Test kits are available to registered clients upon request. We keep packaging to a minimum, so our kits are small and easy to store. Please specify the quantity of each type of collection kit required (whole blood (dried blood spot), serum, or urine) on the following form. You may also order a quantity of information packets, which include comprehensive literature on our services, and our popular Foodstats Food Family booklet. Please note that our standard serum kits cannot be used for our COVID-19 Antibody Test. You can request specimen collection kits for the COVID-19 Antibody Test below, in addition to collection kits for our other tests.
Our test kits are shipped free of charge via USPS or FedEx ground to clients within the US and Canada. Orders shipped outside of the US and Canada are shipped via Fedex Express with costs billed back to the client. These fees vary based on the weight of the shipment. Please allow up to two weeks for fulfillment.
