Request Kits

Test kits are available to US BioTek Practitioners, free upon request. 

We keep packaging to a minimum, so our kits are small and easy to store. Please specify the quantity of each type of collection kit needed on the below form. 

Our test kits are shipped free of charge via USPS or FedEx ground to clients within the US and Canada. Orders shipped outside of the US and Canada are shipped via Fedex Express with costs billed back to the client. These fees vary based on the weight of the shipment. Please allow up to two weeks for fulfillment.

We reserve the right to cap or decline orders based on unverified or delinquent account status.
