Request Kits

Test kits are available upon request to providers with a US BioTek account. If you do not have a US BioTek account you can create one here.

We keep packaging to a minimum, so our kits are small and easy to store. Please specify the quantity of each type of collection kit required (whole blood (dried blood spot), serum, or urine) on the following form.  Please contact us at 1.206.365.1256 or cservice@usbiotek.com if you have questions about which kits to order.

Our test kits are shipped free of charge via USPS or FedEx ground to clients within the US and Canada. Orders shipped outside of the US and Canada are shipped via FedEx Express with costs billed back to the client. These fees vary based on the weight of the shipment. Please allow up to two weeks for fulfillment.

We reserve the right to cap or decline orders based on unverified or delinquent account status.
