Avoid Specimen Rejection
Patient Full Name (first & last), and Date of Birth must be recorded on ALL specimen containers.
This information must match exactly that which is recorded on the test request form (Nickname or abbreviated name is not acceptable).
Improperly or partially labeled specimens will be
fully rejected.
