US BioTek Laboratories - Payment and Billing
All US Biotek Laboratories services are offered to health care practitioners authorized to order clinical laboratory testing. Pricing information is available to the authorized practitioner by request.
Payment Options:
- Practitioners or patients may prepay US BioTek directly at the prepay fee with a check or credit card number enclosed with the specimen.
- Practitioners may choose to establish a professional account with a credit card number on file or with net 30-days payment terms*. At this time, service is not available to practitioners in the state of New York.
- The patient may request US Biotek Laboratories to bill insurance. The cost is at full retail price. It is the patient's responsibility to verify with their insurance carriers for coverage of the test(s) ordered. The patient is responsible for any portion of the claim not covered by their insurance company. CPT identifying codes are provided for all US Biotek Laboratories testing services as general guidelines only, for the purpose of claims processing. As correct coding may vary per carrier, appropriateness for their intended use must be confirmed with the payor.
- US Biotek Laboratories is not a Medicare or Medicaid provider, and our tests are not reimbursable by either agency.
* US Biotek Laboratories reserves the right to assign credit limits, and hold reporting of test results for clients
unable to maintain good payment status.
Billing for Specimen Rejection:
Clients are responsible for a fee of $15.00 for any specimen that is rejected due to improper collection.
Billing for Test Cancellation:
Should the practitioner wish to cancel a test order, he/she is responsible for the full cost of the test if the test has already been run. For tests that have not been run, the practitioner is responsible for a $15.00 processing fee.
